Terms of Service
These terms govern use of the IHA Care platform, including our text-messaging (SMS) program.
These terms are subject to review by legal counsel and may be updated as the service, vendor relationships, and messaging program develop. Material changes will be reflected on this page.
The service
IHA Care is a care-management platform operated by Institute for Human Advancement, an affiliate of the Institute for Human Advancement (IHA), used by federally qualified health centers (FQHCs), community health centers, and primary care groups to run Medicare care-management programs (such as CCM, APCM, CHI, and PIN) — enrollment, consent, care plans, billable time, and coordination with enrolled members, including via SMS.
Acceptable use
The platform is for authorized navigators, care team members, and enrolled program members. You agree not to use the platform to send unsolicited messages, to misrepresent yourself, to attempt to access accounts or data you are not authorized to access, or to interfere with the platform's operation or security.
SMS / text messaging program terms
By providing your mobile number and consenting through your care navigator's guided disclosure, you agree to receive text messages from the IHA Care program related to your care — check-ins, appointment reminders, and follow-ups.
- Message frequency varies based on your care plan and is not fixed.
- Message and data rates may apply.
- Reply STOP at any time to opt out; reply HELP for help.
- Opting out of SMS does not affect your enrollment in, or receipt of, care-management services.
- Carriers are not liable for delayed or undelivered messages. Supported carriers are not guaranteed to include every carrier.
Not a substitute for emergency care
The platform and SMS program are for care coordination — they are not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911 or your local emergency number.
Disclaimers and limitation of liability
The platform is provided “as is” and “as available.” To the fullest extent permitted by law, Institute for Human Advancement disclaims warranties of any kind, and is not liable for indirect, incidental, or consequential damages arising from use of the platform or the SMS program, including delayed, undelivered, or misdirected messages.
Changes to these terms
We may update these terms from time to time. Material changes will be reflected by updating the “Last updated” date below.
Governing law
These terms are governed by the laws of the State of New York, without regard to conflict-of-laws principles.
